The Housing Initiative is intentionally limited in scope to ensure clarity, safety, and long-term sustainability. It exists to provide housing only — without overlapping into medical or social services.
PROGRAM SCOPE
Non-Clinical Transitional Housing
The Housing Initiative provides short-term lodging for medically stable individuals or caregivers who are unable to safely return home following discharge. Housing is offered by referral only and is intended to support safe transitions during defined periods of need.
PROGRAM BOUNDARIES
Clear Limits, Defined Responsibility
The initiative does not operate as a shelter, treatment program, or care facility. It does not provide medical care, counseling, transportation, or case management. These services remain the responsibility of referring institutions or partner organizations.
ELIGIBILITY
Defined housing for short-term transitional needs
The Housing Initiative serves medically stable adults and caregivers who are unable to safely return home following hospital discharge or during periods of active treatment. Housing is intended for short-term use only and supports defined transition periods rather than permanent residency.
REFERRAL PROCESS
Structured access through institutional partners
Access to the Housing Initiative is coordinated through hospitals and approved partner organizations. These partners remain responsible for clinical care, case management, and ongoing support services throughout a resident’s stay.
All residents are screened and accepted exclusively through referral partners. The initiative does not accept walk-ins, self-referrals, or emergency placements.The church provides housing only and does not deliver medical, caregiving, counseling, or transportation services onsite.
